Responsibilities

• Manages consumers on assigned caseload and provides a variety of supportive services

• Assists consumers in the coordination of medical appointments and linkage to all applicable benefits

• Ensures consumers meet the supportive housing program guidelines

• Provides Illness Management Recovery services and continuous medication management

• Provides supervision and performs crisis intervention services as needed

Qualifications

• BSW in social work, BA/BS in psychology

• BA/BS in unrelated major, with two years of experience in mental health

• Valid New Jersey driver’s license
